About our Department:
CPP offers comprehensive Health Services for all students in the District. Each elementary health office is staffed by a Registered Nurse. The middle and high school health offices are staffed by a combination of Registered Nurses, Licensed Practical Nurses, and a Certified Nurse Assistant.
COVID Test Kits: Families are welcome to inquire about picking up a COVID test kit from their child's school Nurse Office. Contact your school for more information.
Emergency contact information: Please make sure that your child’s home school maintains current contact and pick-up information, in case of illness or an emergency.
Bee Sting Allergies, Food Allergies, Seizures, Asthma, Diabetes, and other medical conditions: If your child has any of these or other medical conditions, please contact your child’s school Health Office to discuss their medical history and treatment plan. This is vitally important, as time is of the essence in treating many these conditions.
If your child is ill, it is in his/her best interest and that of others to stay at home. If, based on the assessment of the School Nurse, your child is too ill to remain in school, you will be requested to pick up your child, as soon as possible.
Medication in School: NYSED regulation requires a physician’s order for all medication that is taken at school, including any over-the-counter medication, such as Tylenol or Advil. A new order must be provided every school year. The parent/guardian shall supply the medication ordered.
Immunizations: (Click Link to left) NYSDOH requires all students must be completely immunized for school attendance following the ACIP immunization schedule(s).
Physical Education Excuses: If your child is unable to participate in physical education or is restricted from certain activities, a doctor’s note must be sent to the Health Office.
School Physicals: Students entering grades PreK or K, 1, 3, 5, 7, 9, and 11 and any new student to the district are required to have a physical.
Routine screenings: NYSED Law §905(1) requires new students and students in grades K, 1, 3, 5, 7, and 11 to be screened for vision and hearing, as well as for scoliosis in grades 5, 7, and 9. Color perception screening occurs upon entry to the district.
Athletics: Students interested in playing scholastic sports must register online with FamilyID. A current physical exam (within one year of the start of the sports season) is required. Sports physicals are held several times during the school year. A medical eligibility update form is also required, if the physical is more than 30 days old.
Working Papers: Students must complete the application for employment certificate, have a current physical exam (within one year of the start of employment), and evidence of age. Questions can be directed to the middle and high school health offices.
BMI data reporting: NYSED law requires that BMI and weight status group be included as part of the student’s school health examination. All school districts must take part in a BMI online survey conducted by the NYSDOH. If you have questions about this survey, please contact your child’s school nurse.
